DESCRIPTION OF INDIVIDUAL: 29-year-old male who is friendly and outgoing. He enjoys being around others. It is important to give him time to respond to questions as it may take him some time to answer. He enjoys having conversations and will laugh at jokes. He has an unsteady gait, becomes weak and may need assistance with long distances. He can get physically tired quickly. It is important for him to have his walker for support.
PROVIDER REQUIREMENTS: Job coaching for independent employment at Lowes in Mentor. Two days a week. Assistance in learning new skills/tasks within the store. Assistance with communication with his employer.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Cerebral Palsy, Hip Dysplasia’ Sleep Apnea and Cognitive delay.
HOUSING: Live in Painesville/Concord area.
