DESCRIPTION of INDIVIDUAL: The individual is friendly with everyone and is nice and easy going. He’s difficult to understand at times. He’s polite and genuine. The individual has a great sense of humor. He loves sports and loves work and being involved in his work. He enjoys going out into the community such as to the movies and out to eat.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Individual has mild ID, asthma controlled, diabetes.
HOUSING: The individual lives with his sister and brother in law.
DAY PROGRAM: The individual goes to a Day program.
SUPPORTS NEEDED: Assisting the individual in community outings such as going to the movies or out to dinner or lunch and or meeting with his girlfriend.
