DESCRIPTION OF REQUESTED SERVICES: Seeking HPC/respite services Saturdays and Sundays only, 5-6 hours per day (approximately 9:00 am – 3:00 pm). Majority of HPC/respite to be provided in the community.

DESCRIPTION OF INDIVIDUAL: Individual is a 17 y/o female looking for ways to actively engage in the community on the weekends. Individual’s interests include the zoo, going to the movie theater, painting, drawing with gel pens, and engaging with other crafts, playing 1-on-1
basketball, swimming, bike riding, and all things “princess” and Disney-themed. The Individual is toilet-conditioned, but she requires verbal prompts to remind her to use the restroom. She wears an adult brief in case of accidents and would need assistance to change into a clean brief if soiled. She requires physical assistance with wiping after toileting and verbal prompts to wash her hands with soap and water. Total assistance is needed to make purchases for foods/snacks or other items. The Individual does not have pedestrian safety skills and requires Visual Supervision while in the community; Close and Constant Supervision while crossing streets. Verbal prompts are needed to remind Individual not to engage in excoriation behaviors. At the end of Sunday’s HPC/Respite time, family would like provider to assist with showering. The Individual can physically step in/out of shower independently, but requires verbal prompts to wash her body, and physical assistance to shampoo her hair. Individual can then dress herself
once her shower is complete.
The Individual thrives from praise and at times needs positive reinforcement to engage or continue with a task. She often seeks out verbal confirmation that she is doing something correctly. She may become overly stimulated by noise, crowds, or extreme temperatures, is sensitive to others’ volume and tone of voice, and is self-conscious of her various needs/abilities. If she is over stimulated, feels self-conscious/anxious, or frustrated, then she may exhibit behaviors. History of behaviors include slapping/swatting, yelling, swearing/name-calling, kicking, pulling hair, slamming objects down or breaking objects. Utilizing co-regulation skills, providing reminders to drink water, and engaging in a preferred/calming activity help the Individual return to baseline. Individual’s seizures are typically well-controlled with medication. However, she needs to be
monitored for seizure activity and medication-certified provider will need to administer nasal spray rescue medication for prolonged seizure activity should one occur.
PROVIDER REQUIREMENTS: Family only wants a FEMALE provider; Medication Administration Certified provider for seizure rescue medication; prefer to have a provider who is upbeat and energetic.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Hypoxic Ischemic Encephalopathy; Focal Epilepsy; Moderate Intellectual Impairment (FSIQ 42); Autism Spectrum Disorder; Tremors; Chronic Insomnia; Cortical Visual Impairment; Glaucoma
HOUSING: Individual lives at home with family in the Concord area.
DAY PROGRAM/EMPLOYMENT: Enrolled in school
