DESCRIPTION of NEEDED SERVICES: Individual is in need of Residential Respite, after day program, while father is at work, Monday-Friday 2:00pm-5:00pm (flexible) and (Saturdays begin in March, flexible). Staff provide communication and general supervision in the home, help prepare meals. Staff will monitor for seizures and follow seizure protocol, though rare.

DESCRIPTION of INDIVIDUAL: Individual is a sweet 37-year-old male who resides with his father in Mentor home.
PROVIDER REQUIREMENTS: This provider will deliver ISP services as noted above.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Autism, ID, depression, anxiety, verbal and motor tics, impulse control, seizures.
HOUSING: Individual lives in his family home with his father.
