DESCRIPTION OF REQUESTED SERVICES: This 48-year-old male is looking for a congregate setting to move into in Lake County. He has some alone time available and enjoys utilizing this when no activities are scheduled.

DESCRIPTION OF INDIVIDUAL: He is routine based, preferring certain activities at certain times of the day, however, is able to be redirected if his schedule changes. He is very kind, and he loves to talk about space and interdimensional portals. He enjoys going out to do different activities and would do so almost every day! He has a history at home of threatening to run away when frustrated and he has gone into the woods behind his family home, however, family has never been concerned as he knows the area well enough and never wanders far. He requires verbal assistance with tasks and reminders to eat healthy.
PROVIDER REQUIREMENTS: He requires assistance with medication administration. He is able to take his own meds but has someone place them in a cup for him. Med certification 1 required.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Autism spectrum disorder, borderline ID, Schizoeffective Disorder, manic depressive disorder, diabetes.
HOUSING: N/A
DAY PROGRAM/EMPLOYMENT: He is currently looking to identify a new day program.
