DESCRIPTION OF REQUESTED SERVICES: Individual is 27-year-old male who lives in Painesville. He requires physical assistance with ADL’s. He is looking for HPC services every other week. Days needed are Friday (8am-6pm) and Saturday (9am-7pm). Individual is easy going, has a great sense of humor, and kind. Looking for a provider who is a non-smoker and engaging. The family has cats and four dogs. Provider must have a clean background check.

DESCRIPTION OF INDIVIDUAL: Individual is funny, kind, and easy going. He gets along well with others and is engaging. Individual utilizes a power wheelchair for mobility. Individual needs support with bathroom tasks (brushing teeth, bathing, using bathroom), mealtime, getting ready, laundry, cleaning, and transportation. Individual enjoys video games, going for walks, and reading his bible. He has a trained service dog and other animals in the home.
PROVIDER REQUIREMENTS: HPC/Transportation. A provider that is comfortable with animals.
TRANSPORTATION NEEDS: The family has a modified vehicle the provider could use. The hours of HPC would include transportation to appointments.
DIAGNOSES: CP (Spastic Quad), Seizure disorder, Scoliosis, Congenital Hip Dysplasia
HOUSING: Painesville, OH.
DAY PROGRAM/EMPLOYMENT: Individual is employed in the community.
