DESCRIPTION OF INDIVIDUAL: Individual is an 11-year-old female with a primary diagnosis of cerebral palsy and a history of seizures. Individual uses a power wheelchair for mobility and communicates verbally. Individual loves the color purple, listening to music, coloring, arts/crafts, and watching YouTube.
PROVIDER REQUIREMENTS: Provider needs to be able to manually lift and assist with transfers.
