DESCRIPTION OF INDIVIDUAL: Individual is a 26 year old male who attends a VocHab program 5x per week but is looking to replace 2 days with ADS. Individual is very social and enjoys talking and interacting with others. It is very important he has a routine and structure to his day. The individual and his family are looking for a program with a sensory room.
TRANSPORTATION NEEDS: Individual already has transportation.
DIAGNOSES: Developmental Disability, Autism Spectrum Disorder, ADHD, Others specified delay, other closed fractures of distal end of radius, encopresis, hearing loss (conductive), optic disc anomaly (congenital), pectus excavatum, joint laxity of left hand, joint laxity of right hand, juvenile idiopathic scoliosis of thoracic region,
HOUSING: Individual lives in a home with his parents in Willoughby
DAY PROGRAM/EMPLOYMENT: Individual currently attends Voc Hab 5x per weeks.
