DESCRIPTION of NEEDED SERVICES: Looking for about 15 hours HPC services weekly to assist in taking individual into the community and engage in social interactions with others/attending events and activities that might be interested in/ and meet any personal needs in the community. Individual requests a female provider.

DESCRIPTION of INDIVIDUAL: She is 18 years old, and lives in her family home. She enjoys music, games and technology. She sometimes prefers to communicate electronically such as texting. She is verbal and will express her likes and dislikes. She is interested in community involvement and is currently job seeking.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. For any willing and able provider to be accepting of her gender identity. Verbally encourage the uses of self-calming techniques if she is anxious.
DIAGNOSES: Chronic Kidney Disease, Ehlers Danlos Syndrome, Renal Agenesis, Autism, ADHD, Dysantomia (temperature regulation), Chiari 1 Malformation, Mild Hypotonia, Nocturnal Restless legs, Mosaic Pigmentation, Hypohidrosis, Rathke’s cleft cyst, Asthma, Polydipsia, Eczema, Macrocephaly, Heart Murmur, GERD, Sleep Apnea, Constipation, Intermittent Cyanosis, Anxiety, Dyslexia, Kyphoscoliosis, Dysphagia.
HOUSING: Live with mother and father as well as three other siblings in family home in Concord.
DAY PROGRAM: Home school
SUPPORTS NEEDED: HPC in the community/ HPC transportation (non-modified).
