DESCRIPTION OF INDIVIDUAL: Female individual. 71 years old. She is a very positive and social person who enjoys listening to music and relaxing in her room.
PROVIDER REQUIREMENTS: Individual requires total assistance with all ADL’s and medication administration. Foods need to be cut, and she needs reminders to slow down when eating. Individual can walk, but needs physical assistance with navigating her home, stairs and uneven terrain. Individual has a dog and would need a provider comfortable with animals.
TRANSPORTATION NEEDS: non-modified
DIAGNOSES: Moderate Intellectual Disability, Legal Blindness, Lupus, Anxiety and Depression
HOUSING: Individual resides in Eastlake with her sister and brother-in-law
