DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a new staffing agency for HPC hours/congregate waiver home. Female only. Prefers to be in a location with one female (willing to seek out apartment with supports).

DESCRIPTION OF INDIVIDUAL: Individual is a 42-year-old female. She is a strong communicator. She enjoys making money and using her money for things she enjoys. She enjoys having nice nails, buying clothes, and Disney themed things. She also has two small dogs and enjoys caring for them. Individual needs encouragement to make healthy choices for her diabetes and medical needs. She benefits from support to schedule and attend appointments/prioritize doctor recommendations. She can complete most tasks but does not always self-initiate/needs prompting to keep a clean space and improve hygiene practices. She is a smoker. She has alone time (12 hours at home and up to 7 in the community). She is able to respond to emergencies with limited support, use a phone, and voice her needs. She needs assistance to budget her funds and has a guardian/rep payee.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified vehicle.
DIAGNOSES: Diabetes, frequent UTIs, lactose intolerance, migraine headaches, restrictive lung disease. She has been diagnosed with Schizoaffective Disorder, Borderline Personality Disorder, developmental delays, bipolar disorder, Depression, and Arthritis.
HOUSING: Seeking new housing options
DAY PROGRAM/EMPLOYMENT: Attends ADS/VocHab program in Mentor/looking for community employment.
