DESCRIPTION OF REQUESTED SERVICES: Looking for an LPN or RN on Monday, Wednesday, and Friday. The hours needed are 7AM to 5PM. Located in Perry, Ohio. The family has two dogs.

DESCRIPTION OF INDIVIDUAL: Individual is a male in his thirties. Individual enjoys going on his computer, watching movies, getting fresh air, and going out in the community. Individuals use a power wheelchair for mobility. Individual requires support with suctioning, medication through g-tube, hygiene tasks, range of motion, and transfers. *Individual is on TPN but only administered by guardian at night.
PROVIDER REQUIREMENTS: Nursing (LPN or RN)
TRANSPORTATION NEEDS: Modified
DIAGNOSES: Individual is diagnosed with the following: cerebral palsy, short bowel syndrome, seizures, osteogenesis imperfecta, acute urinary retention, cellulitis, quadriplegic, exotropia, moderate episodes of recurrent major depression, myopia of both eyes, ADHD, and obstructive sleep apnea
