DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an HPC provider to assist him in engaging in his community a few days a week. He requires transportation to and from the community in a non-modified vehicle, but the vehicle must be able to accommodate transporting his wheelchair. The individual can independently transfer from his chair to a vehicle and vice versa. He requires general supervision while in the community.

DESCRIPTION OF INDIVIDUAL: The individual is a 37-year-old male, diagnosed with Cerebral Palsy, who lives in his family home in Perry, OH. He enjoys a large repertoire of things, such as reading, learning about anything history related, bird watching and visiting libraries. He is most interested in karate and Pokémon and would love to find a provider who can engage in conversation and activities that pertain to these things. The individual requires transportation in a non-modified vehicle that can accommodate transporting his chair, he requires assistance to make purchases and is independent with all ADL’s. He is rather shy, but once he becomes comfortable with someone, he is very relaxed and laid back.
