DESCRIPTION of NEEDED SERVICES: Individual is interested in drop-in HPC services in her home in Concord. She is looking for someone to come to her home in the mornings and assist her with getting ready for day program. She is looking for service on Monday through Friday 6:15am-8:15am.

DESCRIPTION of INDIVIDUAL: Individual is a 58 year old female with Intellectual disability and cerebral palsy. She is easy to get along with. She utilizes a communication board to communicate and utilizes a wheelchair for long distances.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Severe Intellectual Disability; Lymph edema right leg, left ankle; Mild Cerebral Palsy; Neurotic Excoriation; Left Eye Blindness, Microcephaly, cataract in left eye, Osteopenia, High Cholesterol
HOUSING: Individual lives in her family home with her sister.
DAY PROGRAM: ViaQuest in Painesville
SUPPORTS NEEDED: Individual needs assistance during emergencies. Individual utilizes a wheelchair for long distances. Individual also utilizes a walker for shorter distances. Individual has history of falls. She is blind in her left eye. Has high cholesterol. Has chronic constipation. She has eczema, back, and right knee concerns. Individual is unable to self-administer her medications. Individual needs physical to total assistance with ADL’s.
