DESCRIPTION of NEEDED SERVICES: The individual is a 21-year-old male who lives in Mentor. Looking for a nursing provider, either agency or independent, for DD waiver nursing or private duty nursing. The individual previously had an independent nurse who was authorized for services but needed to step away. The team is looking for a nursing provider to establish nursing services by meeting with the individual and their family, completing a 485, and other relevant paperwork. The individual is diagnosed with absent seizures with a past of grand mal seizures. His seizures are currently well controlled with an absent seizure about once an hour. He has a developmental delay, is non-verbal, and is wheelchair-bound. Occasional constipation, with an allergy to acetaminophen.
SEEKING AGENCY OR INDEPENDENT PROVIDER: The team is fine with either an agency or an independent provider.

DESCRIPTION of INDIVIDUAL: The individual is nonverbal and wheelchair bound. He has frequent seizures and requires constant visual supervision. This individual enjoys watching cartoons on television.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. This individual is looking for assistance with seizure management, bathing, changing clothes, medication administration, and meals.
TRANSPORTATION NEEDS: Modified vehicle.
