DESCRIPTION of NEEDED SERVICES: Individual needs non-modified NMT transportation to/from day program (8:30-2:30) on Tyler Blvd. in Mentor 3 days per week (Tues, Wed, Fri) and transportation to/from work (9-1pm) on Auburn Rd. in Painesville 2 days per week (Monday and Thursday).

DESCRIPTION of INDIVIDUAL: A 41-year-old male who is caring and has a great personality and smile but has difficulty with effective communication, which can cause upset. Has a communication device to assist with communication but needs reminded to use it. This individual likes attention from non-peers and can become temperamental when he does not get his way due to having many other passengers. Likes watching WWE, loves music and dancing, and activity in the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. This individual does best with a small number of other passengers or no other passengers.
DIAGNOSES: Down Syndrome, high cholesterol and acid reflux. Allergies: Augmentin (severe), dust, cats, pollen, smoke
HOUSING: Lives in congregate setting in North Mentor.
DAY PROGRAM: Tues, Wed, Fri on Tyler Blvd. Work in Mentor, Mon, Thurs on Auburn Rd.
SUPPORTS NEEDED: NMT transportation with small number of passengers or no other passengers.
