DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 51-year-old male who is very friendly. He resides with his mother and father in Willoughby and is looking for periodic residential respite services depending on parents’ schedule. Out of the home respite.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
