DESCRIPTION OF INDIVIDUAL: She is verbal but can be very soft spoken. She likes to read, write, spend time with family and friends, loves learning activities, arts and crafts and likes to listen to music. She requires auditory supervision in the day program and visual supervision in the community. She likes to participate in group volunteer activities in the community
PROVIDER REQUIREMENTS:
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Down’s Syndrome, Complete AV Canal Defect repair with residual Mitral Regurgitation, Congenital Hypothyroidism without goiter, Periodontal Disease, right side peripheral vestibular hypofunction with right beating nystagmus, Strabismus/accommodative esotropia, Amblyopia, Eczema, Constipation (monitor and chart daily), High Cholesterol, and slight hearing loss in both ears and Tinnitus.
HOUSING: Lives at home with her parents
