DESCRIPTION of NEEDED SERVICES:  This individual’s guardian is seeking Environmental Accessibility Adaptations to reduce risk within the home to the individual.  The guardian would like to seek solid wood doors to be installed in rooms where the individual frequents when she is upset, such as the individual’s bedroom, to lessen the risk of harm should the individual attempt to punch/hit the existing hollow wood doors when upset.

DESCRIPTION of INDIVIDUAL:  This individual is a 6 year old diagnosed with Autism who lives with her family in Painesville, OH.  She is very creative and curious, and excels in reasoning tasks.  She lacks awareness of danger which poses a risk given her exploratory habits.  She may engage in property destruction if bored, and self-injury if upset.  She is rarely aggressive, though may defend her food or lash out if redirected from self-injury.  She has many effective strategies for self-regulation and learns new tasks quickly.  She is very personable and friendly with her family and siblings.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. 

DIAGNOSES:  Autism Spectrum Disorder

HOUSING:  With family in Painesville

DAY PROGRAM:  School-age

SUPPORTS NEEDED:  Open/close sensors for external access points to the home (windows, doors), others identified via consultation with guardian
