DESCRIPTION of NEEDED SERVICES: This individual is seeking a NMT driver and/or a Self-Directed Transportation provider for transportation to work. She works for the Rocket Mortgage Field House and works most home games and concerts. She also works for the Browns and would need transportation to the stadium for home games.

DESCRIPTION of INDIVIDUAL: This person is a 30 year old verbal female diagnosed with TBI. She is very outgoing and independent. She is looking for a transportation to assist with access to the community and recreation, as well as work.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. A provider should ideally be able to provide one or both of the requested transportation options.
DIAGNOSES: TBI, asthma, Hx of seizures, ADHD, hair loss
HOUSING: Resides with parents in Willoughby
DAY PROGRAM: N/A
SUPPORTS NEEDED: Supervision for pedestrian safety, assistance with observing boundaries, transportation between locations
