DESCRIPTION of NEEDED SERVICES: Individual is a 17 year old male with autism who will soon be turning 18. Upon turning 18, he will have need of a provider to act as his Representative Payee.

DESCRIPTION of INDIVIDUAL: Individual is fully verbal and largely capable of self-determination, but prone to impulse and does not demonstrate knowledge of budgeting. His team has identified that he would benefit from a Money Management provider to assist with budgeting.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, ADHD, Insomnia, Enuresis, Reactive Attachment Disorder, Fetal Alcohol Syndrome, Constipation, and by history Unspecified Schizophrenia spectrum and other psychotic disorder, Disruptive Mood Dysregulation Disorder and PTSD
HOUSING: Currently resides in Residential Treatment
DAY PROGRAM: Individual is in school
SUPPORTS NEEDED: Management of finances and benefits
