DESCRIPTION of NEEDED SERVICES: Individual is a 12 year female seeking drop in HPC services to be provided in her home in Painesville as well as in the community.

FUNDING SOURCE: Level One Waiver

DESCRIPTION of INDIVIDUAL:. Individual is primarily interested in having someone able to take her into the community to access recreation type outings.  Individual is interested in accessing the LCBDD Recreational Brochure.  She also enjoys going to parks, swimming, playing on her trampoline, going shopping at Target, and doing her hair, nails, and makeup.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. 

DIAGNOSES: Mild I/D, Alcohol Related Neurodevelopmental Disorder, ADHD, ODD, and Disruptive Mood

HOUSING: Individual lives in a single family home in Painesville with her mother and brother.  

DAY PROGRAM: Individual attends LaMuth Middle School

SUPPORTS NEEDED:  Individual is independent with all ADLs and typically requires verbal prompts and reminders for completion of most tasks.  Individual struggles with memory related tasks, and it is best to give her one step at a time.  Individual is a visual learner and prefers to learn new tasks on her own, at her own pace, and through videos if possible.  Individual requires total assistance of management of money and to complete desired purchases.  Individual can be overly friendly in the community.  It is important that staff are vigilant with supervision to ensure that she is not oversharing information, wanders/ leaves a designated area, and is having safe conversations with others.  Family will manage all medications.  Family is flexible with dates and hours of services.  Family has a close family friend who is interested in applying as an employee for any agency that is considering services for this individual.  Contact information can be provided at time of provider interview.
