DESCRIPTION OF REQUESTED SERVICES: Individual in need of HPC in an established congregate setting. Individual has no alone time, and needs verbal to physical help with hygiene routine, preparing meals, cleaning and laundry, making purchases, as well as accessing his community and preferred activities. Individual needs licensed staff to administer medications 2x a day.

DESCRIPTION OF INDIVIDUAL: Individual is a 61-year-old male. He loves Cleveland Sports and spending time with his family. He needs encouragement to make good choices and not take things that do not belong to him.
PROVIDER REQUIREMENTS: Needs Med Cert 1 certified staff
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Moderate ID, Down Syndrome, Hypothyroidism, Vitamin D Deficiency, High Cholesterol, Lactose Intolerant, Depression, and Arthritis.
HOUSING: Seeking HPC services in established congregate setting. Prefers to be in Western Lake County (Mentor, Willoughby, etc.)
DAY PROGRAM/EMPLOYMENT: Individual attends ADS program, 5 days a week, M-F.
