DESCRIPTION of NEEDED SERVICES: HPC with housing 24/7 and On Site/On Call overnights

FUNDING SOURCE: IO Waiver

DESCRIPTION of INDIVIDUAL: Individual is a 61 year old male seeking 24/7 residential services to be provided in a new residence.  Individual requires HPC during all waking hours with a minimum of auditory supervision both in his home and in the community and OSOC during bedtime.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. 

DIAGNOSES: Diagnostic history includes Bipolar Disorder Schizoaffective Type, Pervasive Developmental Disorder, and Mild-Moderate Intellectual Disability. Medically, individual is treated for Diabetes, Hypothyroidism, Hypertension, Hyponatremia, and Hyperlipidemia.

HOUSING: Client is seeking a new home, preferably in Mentor, Mentor on the Lake, Kirtland, or Willoughby.  Client is interested in either a first floor apartment or a single family home

DAY PROGRAM: Viaquest Day Services, Wednesdays Only

SUPPORTS NEEDED: Individual requires verbal prompts with most ADLs, but physical assistance with more complex tasks such as cooking.  He requires total assistance with management of finances, scheduling and attending all doctors’ appointments, and requires med certified staff.  Individual enjoys listening to music, going out to eat, and watching sports games on TV.  Individual prefers a quiet environment.  He enjoys having his staff around and will go to them with any concerns that he has.  Individual is seeking one roommate to live with him, male preferred.  Individual has some mobility limitations due to his age.  He prefers a first floor apartment or ranch style home as he struggles with stairs if he were to have to use them frequently.  He will also need transportation to access the community.
