DESCRIPTION of NEEDED SERVICES: Adult Day Support, 5 days/week. Will require verbal to physical assistance with some ADL’s, including verbal to physical assistance preparing meals and with hand washing, and physical assistance with zipping liner into his jacket and dressing appropriately for the weather. Will also need transportation in non-modified vehicle to and from program.

DESCRIPTION of INDIVIDUAL: 24 year old male with Autism, non-verbal – uses AAC communication device. Story boards and schedules are very important to him.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, Epilepsy, Intermittent Explosive Disorder, IBS, Excessive Weight Gain, Pervasive Developmental Disorder, ID, Constipation, Migraines, Accommodative Component Esotropia, Pilonidal Cyst without Abscess
HOUSING: Individual resides in a home with his mother in Mentor, Ohio.
DAY PROGRAM: Recently stopped attending a day program in Mentor, Ohio that he was attending for 5 days/week
SUPPORTS NEEDED: Auditory supervision. Provider will be responsible for social interactions and community integration.
