DESCRIPTION OF REQUESTED SERVICES: The individual is a 17-year-old male who is wheelchair dependent and unable to bear weight. He lives with his family in Painesville. Currently his family has to carry him from his bedroom into the bathroom to bathe. They also have to pick him up to put him into his bed. The family would like to explore a track system, at least in his bedroom, to get into bed and into a shower chair before bathing. They may also like to explore bathroom modification options.
