DESCRIPTION of NEEDED SERVICES: This individual is a 33-year-old female who is seeking HPC transportation to/from her home in Willoughby (located in Tamarac apartments) to Dr appointments, community activities and specifically 12 weeks of bowling league to begin May 5th for 12 weeks at the Mentor Roll House. Drop off at 6:30pm and pick up to home at 10:30pm. Modified vehicle is needed.
