DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC support for assistance with daily living tasks such as meal prep, shopping, grooming, cleaning, transportation to medical appointments and engagement in community activities. This individual prefers a female and someone that is comfortable around her service dog. Individual is looking for support during weekdays—3-4 days per week, around 20 hours each week.

DESCRIPTION OF INDIVIDUAL: Individual is friendly and easy going. She has a service dog that is with her at all times. Individual is able to walk independently but often utilizes a walker or power wheelchair. She does not like loud sounds/noises.
PROVIDER REQUIREMENTS: HPC and HPC transportation
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Mild Cerebral Palsy
HOUSING: This individual lives in a house in Mentor, Ohio with her grandparents and two dogs (one is her service dog).
DAY PROGRAM/EMPLOYMENT: This individual is currently enrolled in college and does not attend a day program or have a job at this time.
