DESCRIPTION of NEEDED SERVICES: This individual is a 30-year-old male with Autism. He resides with his mother and stepfather in their family home in Mentor. He is looking for a HPC provider to help with access to the community and ADL skills around the home.  He needs to be actively engaged while with staff.  Hours flexible, looking for one day a week (weekends okay)

DESCRIPTION of INDIVIDUAL: This individual is a 30-year-old male with Autism. He resides with his mother and stepfather in their family home in Mentor. He is fully ambulatory.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. 

DIAGNOSES: Autism, Moderate ID, Anxiety, OCD, Chromosomal Abnormality Syndrome, Nervous System Anomaly, Functional Dusphagia, Medication Sensitivity (Serotonin Syndrome), Pyrrole Disorder, Immature Adrenaline Response Syndrome, Hx Serotonin Syndrome/medication induced psychosis, PANS, Mold Illness.

HOUSING:  Lives at home with his mother and step-father in Mentor.

DAY PROGRAM: Seeking.

SUPPORTS NEEDED: Visual supervision, transportation as needed.  He benefits from routine and established boundaries, structure and being given tasks that are achievable, and working with consistent staff that are familiar with preventative strategies identified in his plan.
