DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC transportation to and from summer camp. This individual lives in Mentor-on-the-Lake and will attend summer camp on Mondays, Tuesdays, Wednesdays and Thursdays from 6/22/26 to 7/16/26. The summer camp is at Broadmoor school in Mentor, Ohio and drop off time is 9AM and the pickup time is 2PM.

DESCRIPTION OF INDIVIDUAL: This individual is 16 years old. She is non-verbal and utilities a wheelchair to navigate her environment. This individual in not able to bear weight and needs a wheelchair accessible van. She is fine with other individuals being present as well as she currently rides the bus to and from school each day.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Wheelchair accessible vehicle is needed
DIAGNOSES: Cerebral Palsy, developmental delay, Spastic Tetraplegia, Anxiety, and a developmental language disorder
HOUSING: Individual lives with her parents in Mentor-on-the-Lake
DAY PROGRAM/EMPLOYMENT: Individual is a student at Broadmoor school
