DESCRIPTION OF REQUESTED SERVICES:27-year-old male seeking looking for a HPC staff. Individual lives with mom in mentor, Ohio Individual would need assistance communicating his emotions and can sometimes engage in self-injurious behavior by banging his head. He would like staff to take him out into the community and provide assistance in the home 1-2x/week for up to 2-3 hours or more a week.

DESCRIPTION OF INDIVIDUAL: Although he is verbal, he rarely talks, and when he does, it is very quiet. He has difficulty communicating his emotions and can sometimes engage in self-injurious behavior by banging his head. He enjoys going places and doing new things, especially outdoor activities. Does not do well in large loud areas, prefer to stay away from loud noises. Does best with smaller groups.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. It is recommended that a provider have training in a crisis intervention curriculum for support focusing on de-escalation.
TRANSPORTATION NEEDS: Individual requires a non-modified vehicle
DIAGNOSES: Autism, Anxiety, MTHFR mutation
HOUSING: Individual currently lives with his mother in the family home in Mentor
DAY PROGRAM/EMPLOYMENT: Individual currently does not attend an ADS program.
