DESCRIPTION of NEEDED SERVICES: Individual would like HPC services to help with his ADL’s specifically showering up to 2x a week, total 3-4 hours.

DESCRIPTION of INDIVIDUAL: Individual is a 33-year-old male living in Willoughby with his mom. He utilizes a specialized wheelchair and shower chair. He is able to support transfer once he feels stable on his feet.
PROVIDER REQUIREMENTS: A provider will deliver ISP services as determined at the initial Service meeting.
DIAGNOSES: CP, Hearing Impaired
HOUSING: Individual lives at home with mom.
DAY PROGRAM: Does not attend.
SUPPORTS NEEDED: Individual needs assistance transferring from wheelchair onto shower chair. For transfers can bear some weight, he is tall, but very thin. Can transfer independently from bed to wheelchair.
