DESCRIPTION OF INDIVIDUAL: The individual is a 35-year-old male
TRANSPORTATION NEEDS: Provider transportation in a non-modified vehicle
DIAGNOSES: Jason has been diagnosed with Asperger’s, ADHD, Pectus Carinatum, Bipolar Disorder. Jason has difficulty walking due to a significant lower back injury in 2024 while at work. This injury caused severe nerve damage at his Sciatic Nerve which travels down his leg and makes walking very difficult, he currently uses a case and receives physical therapy.
HOUSING: Individual lives with his mother in Painesville, Ohio.
