DESCRIPTION OF REQUESTED SERVICES: This individual is a 57-year-old male who lives with his mother and sister in Mentor. He is looking for a day program where he can participate in activities and attend 5 days/ week.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Adult Day Support program agency.

DESCRIPTION OF INDIVIDUAL: He is verbal and can communicate his wants and needs at a basic level. He needs verbal prompting for most ADLs. He loves to stay up late and sleep in. He does well when he has a place to go to that is quiet during the day. Sometimes he can be over-stimulated with a noisy environment and wants a break from the noise. He loves to read books and listen to music.
PROVIDER REQUIREMENTS: He would need staff certified in med pass as he is unable to administer medications independently. He does not currently take any medication during the day at day program.
TRANSPORTATION NEEDS: non-modified vehicle
DIAGNOSES: Depression, OCD, Downs Syndrome, Moderate ID, Bi-Polar disorder.
HOUSING: lives with family in Mentor
