DESCRIPTION OF REQUESTED SERVICES: Looking for HPC services in the family home. Individuals needs assistance with daily living tasks, transportation and engagement in the community (If needed). Family is looking for Monday- Friday, around 10-15 hours a week and maybe some weekend hours.

DESCRIPTION OF INDIVIDUAL: Individuals first language is Spanish, but he also speaks English. Individuals speak in short, and simple sentences. Sometimes it is helpful to repeat information. Individual needs assistance in making decisions, Verbal assistance with safety drills and in the event of an actual emergency, non-medical transportation to and from the community. Individual would require assistance with cleaning, would need physical assistance with heavy cleaning, and assistance with laundry. Individual likes being outside, coloring, playing the drums, and riding a bike or scooter. Also likes cars, trucks, and school busses, Spiderman and music (mariachi bands, rap, Spanish songs)
PROVIDER REQUIREMENTS: HPC certified
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Moderate Intellectual Disability and ADHD
HOUSING: In family home, in Painesville
DAY PROGRAM/EMPLOYMENT: Attends Voc five days a week
