DESCRIPTION of NEEDED SERVICES: This individual is a 34-year-old female who resides in Mentor. She is looking for HPC services (preferably in the evening and/or weekend) for community access and activities. Up to 8 hours per week, times to be determined. Additional support may be needed on days when her work program is scheduled to be closed.

DESCRIPTION of INDIVIDUAL: This individual is safest within visual supervision in the community. She is verbal and will let you know her likes and dislikes. She enjoys a variety of activities such as spending time with her family, listening to music, watching TV/movies, and going out to eat.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non modified vehicle
DIAGNOSES: Cerebral palsy, severe ID, Seizure disorder
HOUSING: She resides at home with her parents in Mentor.
DAY PROGRAM: She attends a day program/voc hab five days a week
SUPPORTS NEEDED: Individual would like assistance finding new activities and socializing. She would need physical assistance with uneven or icy/wet surfaces and stairs, as well assistance with purchasing in the community.
