DESCRIPTION OF INDIVIDUAL: He is able to communicate verbally, but may repeat what someone says, as he may not understand what is being asked of him. He enjoys anything snoopy, spending time with his family, eating out, playing games, community activity, and taking breaks in his recliner at home. He does have frequent health issues, which may slow him down physically at times.
PROVIDER REQUIREMENTS: ADS certified
TRANSPORTATION NEEDS: Non- Modified vehicle
DIAGNOSES: Moderate range of ID, Impulse control D/O, High Blood Pressure, and Urinary retention.
HOUSING: Lives with family in Mentor.
DAY PROGRAM/EMPLOYMENT: Attends ADS program part-time.
