DESCRIPTION OF INDIVIDUAL: Individual recently graduated from high school and has located housing. She will have 24-hour support when she is not attending her day program. Individual enjoys swimming, going for rides in the community, music, singing, YouTube, dancing, swinging, going to the beach, going out to eat, bowling. She needs assistance with hygiene care, meal preparation/planning, and basic day-to-day care needs/supervision. Individual has limited speech and specific communication style with day-to-day needs/wants (does well with social stories and 2-3 word notes). Individual has a specific routine and needs consistency to ensure a smooth day. Individual has history of physical aggression such as punching, hitting, and pinching others. Individual will engage in SIB such as hitting herself in the face if her routine is disrupted or requests are not met with appropriate tailored responses. She may yell or scream and will also hit her head against the wall/table and/or punch or headbutt holes in walls. Guardian will provide specific training to provider to ensure her needs can be met appropriately. Individual’s school intervention team is actively involved in her care to provide support, specific education and training as needed.
PROVIDER REQUIREMENTS: Provider will need advanced experience with Autism and Crisis Intervention. Med cert I and II.
TRANSPORTATION NEEDS: Non-modified vehicle

DAY PROGRAM/EMPLOYMENT: Attends Day Program M-F
