DESCRIPTION OF INDIVIDUAL: Individual is an 18-year-old verbal female. Individual is seeking a new congregate care setting with 1-3 roommates. Individual is typically independent with mobility, although may require some assistance navigating curbs, steps, stairs, icy and uneven surfaces. Individual loves all animals, especially horses. She enjoys going on walks outside or in stores, coloring and drawing, and going out to eat. Individual requires visual monitoring while in her home and in the community, verbal prompts and reminders to maintain safety skills, and reminders about her routine. Individual requires assistance with meal planning, meal preparation, and household tasks. Individual requires assistance and support getting to and from all medical appointments.
PROVIDER REQUIREMENTS: Crisis prevention and intervention techniques training preferred, Med Certification
TRANSPORTATION NEEDS: Non-Modified Vehicle
