DESCRIPTION of NEEDED SERVICES:  37-year-old female is seeking an agency provider for Homemaker personal care supports. Request of 15 hours per week- some Saturday/Sunday assistance too.

FUNDING SOURCE: Level I Waiver

SEEKING AGENCY OR INDEPENDENT PROVIDER: Open to agency or independent provider

DESCRIPTION of INDIVIDUAL: 37-year-old female who lives in Concord Twp., with her mother and father. She is non-verbal but loves to be around others and engaged socially. She has no mobility concerns. She always requires visual supervision. She struggles with anxiety during times of transition & change. This can make transitions difficult and extremely slow.  She has a history of physical aggression towards staff, or self-injurious actions towards herself during these transitions if not provided with advanced notice of change. Individual benefits from use of a visual calendar and social stories to help transition her to various activities at home and in the community. She loves airplanes & air shows. Her tablet is important to her and she enjoys watching videos and listening to music. Her animals are important to her as is time outdoors swinging her in her preferred swing at home. During time spent with the individual she will require support with Diet, Assistance with Transitions, Assistance with Toileting, Dressing, Assistance with Meal Preparation, Transportation, etc.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.

TRANSPORTATION NEEDS: Non-Modified vehicle. Does Buckle Boss during times of transportation (HRC Approved)

DIAGNOSES: Autism, hypotonic Cerebral palsy, seizure disorder, constipation, and anxiety

HOUSING: This individual lives in Concord with parents.

DAY PROGRAM/EMPLOYMENT:  Attends Day program up to 5 days a week.
