DESCRIPTION of NEEDED SERVICES: Individual is seeking transportation from his home in Willoughby OH to his camp, which is hosted at Broadmoor School in Mentor OH. Primarily seeking morning drop-off by a provider able to do HPC and ATN. Service needed to run between June 22-July 16.

DESCRIPTION of INDIVIDUAL: Individual is a 13-year-old boy with Autism. He is non-verbal but has limited ability to communicate via talker. He does well traveling. He may wander but is not an elopement risk.
PROVIDER REQUIREMENTS: Certified to provide HPC and/or ATN to I/O Waiver.
DIAGNOSES: Autism
HOUSING: Lives with family in Willoughby
DAY PROGRAM: N/A
SUPPORTS NEEDED: Transportation to and from camp. He is able to walk on his own but would need close supervision walking to and from the building as he may wander.
