DESCRIPTION OF INDIVIDUAL: Individual is a 28-year-old male who lives in Painesville, Ohio. Individual is non-verbal and have limited mobility. The individual gets nutrition, hydration, and medication via g-tube. Individuals require total support with ADL’s and transfers.

DIAGNOSES: Cornelia de Lange Syndrome, Gastroesophageal reflux disease without esophagitis, Cyanosis, Apnea, Stereotypic movement disorder, etc..
