DESCRIPTION OF REQUESTED SERVICES: Client is in need of HPC and OSOC services and plans to attend ADS program M-F. Client is interested in residing on her own and/or in congregate setting. Client has an electric wheelchair and needs help with transportation to and from community; events and appointments, physical help using her Hoyer lift; to transfer, help with daily tasks.

DESCRIPTION OF INDIVIDUAL: Client uses a Hoyer lift and has an electric wheelchair. Client is active in the community; interested in Special Olympics. Client struggles with anxiety and getting involved in others information. Client is social and loves her phone and talking to family and friends.
