DESCRIPTION OF INDIVIDUAL: This individual is a 32-year-old male. He is a happy, social and friendly young man with a diagnosis of Nooman’s Syndrome & Failure to Thrive. He is verbal and resides with his mother & father in the family home in Willoughby. He is very outgoing and loves to be around others and stay busy.
PROVIDER REQUIREMENTS: Follow OISP. Looking for a new day program.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Nooman’s Syndrome, Failure to Thrive
HOUSING: Individual lives with his parents in Willoughby, Ohio
DAY PROGRAM/EMPLOYMENT: Currently under Deepwood Industries. On medical leave until health needs are resolved. Needs a day program where attendance is not factor.
