DESCRIPTION OF REQUESTED SERVICES: Individual is a 56-year-old female seeking a provider for ADS/VH and/or GES.  She is open to attending an ADS/VH program M-F but for GES would only be seeking to work about 2 days/week (no longer than 4–5-hour shifts).

SEEKING AGENCY OR INDEPENDENT PROVIDER:  Agency provider

DESCRIPTION OF INDIVIDUAL: Individual had been working independently in the community for the past 25+ years but lost her job.  She recently recovered from hip surgery and has been cleared by her doctor but does need to take breaks periodically throughout the day/while working.  She enjoys repetitive tasks that aren’t too physically demanding.  She enjoys being social and earning a paycheck.  Staff who are familiar with Prader Willi Syndrome is ideal.

PROVIDER REQUIREMENTS: Provider will deliver services as identified in the Individual Service Plan.

TRANSPORTATION NEEDS: Non modified vehicle (family may provide transportation)

DIAGNOSES:  Mild Intellectual Disability, Prader Willi Syndrome, High Cholesterol, Osteopenia, Scoliosis, Idiopathic generalized osteoporosis, severe bone on bone osteoporosis in both hips, affecting the ROM and strength in her hips, severe left genu valgus and bilateral pronation of both feet requiring the use of orthotics at all times for safety when walking, Low vitamin D.  Individual has the following Allergies: Wildflowers, seasonal allergies, latex, red dye, and food additives. She has experienced buckling of her legs due to pain in the hips and knees.

HOUSING: Resides with family in Painesville.

DAY PROGRAM/EMPLOYMENT:  Recently lost her part-time competitive integrated employment.  She would like to seek a provider for ADS/VH and/or GES for socialization, building work skills and earning a paycheck.
