DESCRIPTION of NEEDED SERVICES: Individual is looking for remote supports/assistive technology.

DESCRIPTION of INDIVIDUAL: 29-year-old female that is working on moving out of her parents’ home. She is smart, likes to help others, and is a hard worker. She enjoys Legos, arts and crafts and swimming. She also enjoys being around kids and helping animals. She does not want to be singled out due to her diagnosis of Prader Willi. She is involved in community events and can make her own decisions.
PROVIDER REQUIREMENTS: Remote supports/assistive technology- be able to provide supervision overnight, and mornings prior to her leaving for ADS. Provide technology to lock and unlock fridge/cabinets and storage
DIAGNOSES: Prader Willi and Scoliosis.
HOUSING: Will be living independently in an apartment/or condo in the Willoughby/mentor area.
DAY PROGRAM: Attends ADS five days a week.
SUPPORTS NEEDED: Remote support supervision overnight and in the mornings, verbally remind her of her routine, assist in ensuring locations with food remain secure until mealtimes to ensure her health and safety.
