DESCRIPTION of NEEDED SERVICES: This individual is a 32-year-old male with Autism Disorder, Moderate ID, and ADHD. He is looking for NMT to his ADS (Mentor) from his home (located in Painesville), about 10 miles for the trip. This would be a total of 1 trip per week on Friday morning.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism Disorder, Moderate ID, ADHD
HOUSING: He lives with his parents in Painesville Township
DAY PROGRAM: Currently attends a day program in Mentor.
SUPPORTS NEEDED: Transportation to his day program in a non-modified vehicle.
