DESCRIPTION OF REQUESTED SERVICES: The recommended equipment is a tub transfer shower chair system designed to provide maximum safety, stability, and full-body support for individuals with significant mobility limitations. This system includes a mobile rolling base, a stationary tub base, and a bridge mechanism, allowing the user to be safely transferred into and out of the bathtub without the need for standing or unsafe manual lifting.

DESCRIPTION OF INDIVIDUAL: The individual is a 64-year-old male with a significant medical history. The patient is fully dependent or requires maximum assistance for nearly all activities of daily living, including bathing, dressing, transfers, and toileting. He demonstrates poor sitting balance and is unable to stand independently, placing him at a high risk for falls, as evidenced by a MAHC-10 score of 5/10. Additionally, he has significantly limited strength, particularly on the right side, with multiple muscle groups graded at 0/5, indicating no active movement. These impairments necessitate full support, careful positioning, and caregiver assistance for all functional tasks.
PROVIDER REQUIREMENTS: Provide assistance with learning job skills, and assistance with communication and building rapport with others.
DIAGNOSES: He is Dx with paraplegia, right side hemiparesis, brain injury with prolonged loss of consciousness, aphasia, muscle weakness, chronic fatigue, pressure ulcers, hypertension, asthma, and wheelchair dependence.
HOUSING: Lives with his mother, no pets
