DESCRIPTION of NEEDED SERVICES: This individual is a 33-year-old male with Autism. He resides with his mother and stepfather in their family home in Mentor. He is looking for a VH or GES provider. He learns well and works hard. He is very social and needs to be actively engaged while with staff. Male staff preferred. Visual supervision, transportation as needed, social support. He benefits from routine and established boundaries, structure and being given tasks that are achievable, and working with consistent staff that are familiar with preventative strategies identified in his plan.

DESCRIPTION of INDIVIDUAL: This individual is a 33-year-old verbal male with Autism. He resides with his mother and stepfather in their family home in Mentor. He is fully ambulatory. He learns well with visual modeling and verbal direction, and masters tasks quickly.
PROVIDER REQUIREMENTS: VH certification, NMT preferred.
DIAGNOSES: Autism, Moderate ID, Anxiety, OCD, others
HOUSING: Lives at home with his mother and stepfather in Mentor.
DAY PROGRAM: Seeking.
TRANSPORTATION NEEDS: Non-modified vehicle
