DESCRIPTION of NEEDED SERVICES: Individual is seeking HPC provider during weekday mornings to facilitate community access. She generally likes going for walks, car rides, and shopping trips. Needs assistance with bathroom, eating, and routines. Needs in-view supervision while indoors, close and constant in the community to assist with pedestrian safety.

DESCRIPTION of NEEDED SERVICES: This person is a 31-year-old female who resides in a congregate living home in Mentor. She enjoys flipping through magazines, sensory items, laughing, van rides, going to Starbucks and hanging out in her room. She requires visual supervision due to PICA and past elopement attempts. Having a quiet, calm environment helps along with routine and structure. Knowing what to expect next using the “first, then” format is helpful. Individual cannot be left unsupervised due to not being able to determine between safe and unsafe situations. She needs physical assistance with eating due to choking risk.
PROVIDER REQUIREMENT: Crisis prevention training is a plus. HPC/ATN certification needed.

SUPPORTS NEEDED: Transportation and supervision primarily, assistance with tasks throughout the day as they emerge, based on the setting.
