DESCRIPTION OF INDIVIDUAL: Male, 16 years old. Lives in family home with his mother, father and older sister. Total assistance with ADL’s, currently bedridden due to a recent fall, he will require visual supervision and assistance in all daily living task. Total assistance with transitioning to and from wheelchair, assistance to attend medical appointments, and use of Hoyer lift.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: modified vehicle.
DIAGNOSES: Diagnosed with Duchenne Muscular Dystrophy, Sleep Apnea, Oppositional Defiant Disorder and Anxiety.
HOUSING: In family home in Willoughby.
DAY PROGRAM/EMPLOYMENT: N/A
